It's by Chris Cunningham, one of the more interesting directors of music videos. He did videos for Aphex Twin's Come To Daddy and Windowlicker in addition to videos for Björk and Autechre.

The DVD is on sale in most shops; the cover work is great.

It certainly is. A great mix of disturbing visuals, comedy, and Aphex Twin's music.
